Survey for Health Professionals: Cancer Rehabilitation and Survivorship Institute – Needs Assessment
Dr. Jennifer Jones, and the Cancer Rehabilitation and Survivorship team, invite you to participate in a survey that will inform the development of a multidisciplinary and comprehensive Cancer Rehabilitation Training Institute. To ensure that the proposed Institute meets the needs of its prospective learners, your feedback will inform the development of education courses on topics related to cancer rehabilitation and survivorship.
Title of the project: Cancer Rehabilitation and Survivorship Institute – Needs Assessment
Who is eligible to participate: All interested community healthcare professionals
Your participation involves answering multiple choice questions regarding your preference for several course topics, including preferred format, delivery and length. Completion of this needs assessment will only take about five minutes of your time.
Before consenting to participate, please take the time to read, understand and consider carefully the following information: Your participation is voluntary. You may skip any questions that are not relevant to you and stop the survey at any time. Your identity will not be revealed and answers to questions will be anonymous.
